113 STS-107 Columbia 7 16/01/2003 (01/02/2003) Space Shuttle Columbia disintegrated at around 14:00 UTC, some 16 minutes prior to a scheduled landing at KSC at an altitude of approx. 61 km (200,000 ft) and a speed of Mach 18.1 over north central Texas (32.9 N / -99.0 W, near Dallas). The identified root cause was failure of the vehicle's thermal protection system, triggered by a breach to the shuttle's left main wing. All seven astronauts on the research mission (Spacehab Double Module), including the first Israeli space traveller, were killed when the shuttle broke up.
